Assessment Process:
Filling out forms: Health & Lifestyle Questionnaire, Par Q,
Pre-exercise screening form, medical questionnaire, waiver, medical
consent or referral form if applicable.
Consultation with client on cardiovascular, muscular
strength and endurance, flexibility, body composition and
nutritional goals. Current activities and interests, previous
injuries, problems or limitations to exercise, availability and
REALISTIC time commitment to an exercise program.

Client teaching on: taking pulse, rate of
perceived exertion, talk test, power position and resting heart
rate.

Checking of pulse, blood pressure, and notation of
history of CVD, recent cholesterol, height and weight.

Body composition and girth measurements i.e.:
waist to hip circumference ratio, body mass index, body fat
percentage by digital calipers.

Assessment of postural alignment i.e.: kyphotic,
lordotic, flat back, scoliosis, forward head. Pelvic isolation
assessment. Muscle length/tension relationship (20 separate
tests). Strength, flexibility and cardio endurance tests.

Nutrition forms and log given for clients with
dietary concerns.
